IMPORTANT INFORMATION TO ALL STUDENTS

MEDICAL EXAMINATION RESULTS

All prospective students admitted to the Federal University of Technology and Environmental Sciences, Iyin-Ekiti (FUTES) are required to download the MEDICAL CLEARANCE FORM from their respective Portal account @ https://portal.futes.edu.ng//apply and proceed to any Government Hospital to complete the Medical examination. Students are to bring along the results for authentication at the FUTES Medical and Health Centre.

Students are enjoined to note that the form is already online and downloadable after the payment of the Medical fees and completion of the online Medical Clearance form.

SCREENING REQUIREMENTS

All students are also expected to come along with the following documents:

1. UTME Result Slip
2. FUTES Post UTME Registration Form
3. FUTES Post UTME Screening Result Slip
4. FUTES Admission Slip
5. FUTES Acceptance Clearance
6. FUTES Medical Clearance
7. JAMB Admission Letter
8. Original O level Certificate
9. WAEC/NECO Scratch Card(s), E-Verification may be applicable for candidates using NECO and should be forwarded to admissions@futes.edu.ng
10. Original Birth Certificate/Sworn Affidavit/Declaration of Age
11. Attestation Letter/ Reference Letters from a Clergy or Government officials on Grade Level 13 and above.
12. Twelve (12) Passport Photographs (Write your full name at the back of the passport)
13. Evidence of Payment of Acceptance, Medical Fees (Remita RRR receipts)

Students should come along with three (3) Sets of photocopies of all the above documents.
